Sign-up and Sports Fees

All athletic forms and sport fees should be given to the Athletic Director, coach, or office before the first practice.

1) Register your athletes online

Sports fee will be paid through the TeamSnap Registration at sign-up.

$50 for a FAITH student
$100 for an athlete not enrolled at FAITH (home school, etc.)

FCA can only accept home school students at the middle school level.

2) All students: Turn in the Hold Harmless, Physical or Alternate Year Card, and Parental Consent form to the office.

*Athletes enrolled as students do not need to turn in another Hold Harmless form.

3) High School Students: Turn in the WIAA Eligibility Form

4) Transfer Students: Request an Eligibility Verification of Transfer Student (EVTS) Form
